Consumer Health Data Privacy Policy

Effective September 11, 2026

This policy describes how Schwartz & Associates, PC (the firm) collects, uses, and shares consumer health data through this intake website. It is provided under the Washington My Health My Data Act (RCW 19.373) and similar laws, including Nevada (NRS 603A.400 to 603A.550) and Connecticut.

Consumer health data the firm collects, and why

The assistant asks your permission before asking about your health. If you share health information before you are asked, the firm collects it only as necessary to review the matter you asked it to review, and uses it only for the purposes below. The categories are:

Purpose and use. The firm uses this information only to review whether it can assist you with a potential legal claim, including checking for conflicts of interest; to contact you about that review; to keep records of your consent; and to supervise the accuracy of the intake assistant. The firm does not use consumer health data for advertising and does not sell it.

Sources

The firm collects consumer health data directly from you through the intake assistant and forms on this site, or from a person you authorize to act for you, such as a parent, guardian, or personal representative.

The categories listed above may be disclosed to the recipients described in the next section. Disclosure to co-counsel or referral counsel happens only with your separate consent.

How to make a request. Email lou.schwartz@snalawyers.com with the subject line "Consumer health data request," or call (918) 238-6445. The firm will verify your request and respond within 45 days, which may be extended once by an additional 45 days when reasonably necessary, with notice to you.

How to appeal. If the firm declines to act on your request, reply to the firm's decision within a reasonable time and state that you are appealing. The firm will respond in writing within 45 days. If your appeal is denied, you may contact the Washington State Attorney General at atg.wa.gov/file-complaint, or the attorney general of your state.

The firm will not discriminate against you for exercising these rights.
