Privacy Notice for the Intake Assistant
This notice explains how Schwartz & Associates, PC (the firm) collects, uses, and shares information through this intake website and its AI intake assistant. This notice is the firm's privacy policy for its website and intake assistant. Health information is also covered by the Consumer Health Data Privacy Policy.
Information the firm collects
- Your messages to the intake assistant.
- Form information you enter: name, email address, phone number, state of residence, best time to reach you, and details about your potential claim.
- Health information you choose to share after giving permission.
- Notice and consent records: the notices shown to you, your acknowledgment and consent choices, your electronic signature, and the date, time, IP address, and browser information associated with them.
- Technical information needed to operate and secure the site, such as IP address, browser type, and server request logs.
The site does not use advertising cookies, analytics trackers, or tracking pixels. It loads fonts from Google Fonts, which receives your IP address when your browser requests them.
How the firm uses information
- To answer your questions and operate the assistant.
- To check for conflicts of interest and review whether the firm can assist with your potential claim.
- To contact you using the methods you authorize.
- To supervise the accuracy of the assistant and keep records of notices and consents.
- To protect the site against misuse and to meet legal and professional obligations.
How the firm shares information
- Service providers that process information for the firm under contract: Anthropic, PBC (AI processing that runs the assistant); CITE8, Inc. (the intake platform, a company affiliated with the firm's principal); Vercel Inc. (website hosting and logs); and the workflow, customer-relationship, and email providers the firm uses to receive and manage submissions.
- Co-counsel or referral counsel, only with your separate consent.
- As required by law, court order, or rules of professional conduct, or to protect the rights and safety of any person.
The firm does not sell personal information and does not share it for cross-context behavioral advertising. The firm does not share mobile phone numbers or text-message consent with third parties or affiliates for their marketing.
How long information is kept
Conversations you do not send are not stored by the firm, except that the firm may keep a record of the assistant's responses to supervise their accuracy. The AI provider may keep inputs and outputs for up to 30 days under its own policies, and longer in limited cases such as legal requirements or enforcement of its usage policies. Submitted forms, conflict information, and notice and consent records are kept as long as needed to review your matter and to meet the firm's legal, professional, and recordkeeping obligations; consent records are generally kept for at least four years.
Security
The firm uses reasonable administrative, technical, and physical safeguards. No method of transmission or storage is completely secure, which is one reason the assistant asks you not to send Social Security numbers, account numbers, or documents.
Your choices and rights
- You may ask to access, correct, or delete the personal information you provided through this site.
- You may withdraw consent to calls or texts at any time by any reasonable means, including by telling the caller, replying STOP to a text, calling (918) 238-6445, or emailing lou.schwartz@snalawyers.com. The firm honors revocations within ten business days.
- Residents of states with consumer privacy laws may have additional rights, including the right to appeal a decision about a request. The firm will respond within the time those laws require.

Children
This site is not directed to children, and the firm does not knowingly collect information from children under 13. A parent or guardian should contact the firm directly about a matter involving a minor.
Changes to this notice
The firm may update this notice. The effective date above shows the current version.
